You can register by postal mail or electronic mail. If registering via email, you will need to use a credit or debit card (VISA, Mastercard, Discover or American Express.

Choose your method below.

If registering by mail, and paying by check in US funds :

Print the registration form to fill in by hand or

Fill out registration form using your web browser. Then print the completed form (from your browser window, choose File, then Print)

Mail your form with your payment.

If registering by mail, and paying by credit/debit card ($8.95 convenience feel will be added):

Attach the saved file to your email, and send it
